JOB NAME Ranch Market JOB NUMBER 770T 43622 01 0010 <br /> Work Conducted By i��� r DATE <br /> 1) After arriving on-site, review work order, HASP, and JSA within HASP on Ozone Gas Hazards _ <br /> Note Specific Hazards for conducting working around the offsite wells on second page of work order <br /> 2)- Prior to conducting field work, insert ozone sensitive paper into badge and attach to shirt or coat <br /> Make sure ozone meter has been warmed up Warm up times can be greater than 1 hour <br /> Refer to second page of work order and tilt out requested Information <br /> _ 3) Monitor and document ozone readings outside of ozone compound and ozone panel - - <br /> Since ozone is heavier than air, be sure to monitor for ozone at low points in compound and panel <br /> If positive ozone readings are encountered,call project manager to discuss possible solutions <br /> 4) If no positive ozone readings are found, inspect fittings and tubing connection-for signs of wear or damage <br /> 5) Using meter, Monitor for Ozone 03 along the piping runs, at top of each infection well lid, and within each well box <br /> i <br /> If positive ozone readings are encountered,call project manager to discuss possible solutions <br /> 6) Before leaving the site check your Ozone badge and note badge-color on second page of work order <br /> 7) Call into the Sacramento office before you leave the site <br /> &) Forward field notes and equipment rental forms to Jaff Auchterlonie in Sacramento <br /> mmmc <br /> Site Safety Plan <br /> 03 Meter and Ozone Badge <br /> 112", 9/16', and 15116"sockets, pliers, and other misc tools <br /> Traffic Delineators Nitnle gloves, COC's, drum labels,etc <br />
