2. SCOPE OF WORK CKG will contract with Cascade Drilling,a C57 licensed drilling contractor to install the new MW- 4m(MW-4R)at its present location. The following bullets outline the elements necessary to complete the proposed work: • Obtain a SJDEH Permit for the well installation • Prepare a site-specific health and safety plan for the work. • Mobilize to the site to install the monitoring well. • Develop and survey the monitoring well. The following is a task breakdown of CKG's scope of work. Task 1 —Permitting The SJDEH requires submittal of their"Site Mitigation Well and Boring Permit Application". Supporting documents required with the permit applications include a site-specific health and safety plan;their"Site Mitigation Master File Record Information Form"and this work plan,approved by the RWQCB. Copies of these document are provided in Appendix A. Task 2—Health and Safety Plan A health and safety plan will be prepared prior to initiating fieldwork.The health and safety plan will list potential hazards and protective measures required to protect the field investigation team,other persons on site,and the general public from site specific hazards(Appendix A). Task 3-Subsurface Utility Clearance Because the new MW4R will be installed in the same borehole as the old MW-4,it will not be necessary to clear the borehole for subsurface utilities. Task 4 Well Installation CKG will work with Cascade Drilling to install the new MW-4R to a depth of 85 feet which is approximately 15 feet deeper that the existing MW-5.The well will be constructed through 10 or 12- inch diameter hollow stem augurs.The well will be constructed of 4-inch diameter schedule 40 PVC with 25 feet of 0.02-inch screen from 60-85 feet below ground surface. CKG is proposing a 25-foot screen because water elevation fluctuates widely at this part of the property. Cascade Drilling will start by removing the flush mounted well head and drilling out the existing well. After all the well debris is cleared from the borehole the drillers will advance the hole to the new depth of 85 feet below ground surface.
