Mr. Ross Atkinson <br /> March 30, 2001 <br /> Page 6 <br /> Disposable sampling equipment and personal protective equipment (PPE) will be placed <br /> in plastic bags labeled and stored with the drums. <br /> Sample Handling and Custody Requirements. Samples collected shall be assigned a <br /> unique identifier that will be recorded in the field notebook and permanently attached to <br /> the sample container, either with indelible marker or a gummed label. Each sample <br /> container will be packed in a sturdy shipping container, typically an ice chest. A chain-of- <br /> custody (COC) form, completed in the field and providing a record of the samples <br /> offered for shipment,will accompany each ice chest. <br /> Transfer of samples from the Site to laboratory will follow COC procedures. Sample <br /> transfer custody requires one of the following at all time: <br /> ■ The sample be in a person's physical possession; or <br /> • The sample be in a person's view after he/she has taken possession; or <br /> ■ The sample is secured by a person such that no one can tamper with the <br /> sample; or <br /> ■ The sample is secured in an area that is restricted to authorized personnel. <br /> A properly completed chain of custody record is required to show sample custody <br /> changes from the sampler to the laboratory. When transferring samples, the individuals <br /> relinquishing and receiving them both will sign, date, and record the time on COC. <br /> A COC form,which will be signed by each person when receiving and relinquishing the <br /> samples, will accompany samples offered for shipment. The chain of custody record will <br /> have the following information: <br /> • Sample identification number. <br /> • Signature and printed name of sampler. <br /> • Time of collection. <br /> ■ Date of collection. <br /> ■ Sample matrix. <br /> • Laboratory analysis required. <br /> ■ Signature and printed name(s) of person(s) involved in transfers. <br /> • Inclusive dates of possession. <br /> Samples offered for shipment by freight carrier will be packaged and the COC form will <br /> be signed by the sampler and sealed inside the cooler in a waterproof bag. <br /> Documentation of the carrier will be maintained.
