S A N J 0 A Q I I I Irl Environmental Health Department <br /> C o u N T AMENDED <br /> 7/8/2020 <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> SIMS METAL 1000 S AURORA ST, STOCKTON July 06, 2020 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> (d)A scrap recycling facility that accepts appliances pursuant to subparagraph (B)of paragraph (2)of subdivision <br /> (b)shall provide a monthly report to the department and the San Joaquin County Environmental Health Department <br /> that includes both of the following: <br /> (1)For each appliance received by the scrap facility, the name and address of the person who transported, <br /> delivered, or sold the appliance to the scrap recycling facility. <br /> (2)The total number of appliances received pursuant to the conditions provided in subparagraph (B)of paragraph <br /> (2)of subdivision (b). <br /> 25211.3. <br /> A certified appliance recycler, and any person who is not a certified appliance recycler who is subject to subdivision <br /> (b)of Section 25211.2, shall retain onsite records demonstrating compliance with applicable requirements of this <br /> article and Section 42175 of the Public Resources Code. The records shall be retained for three years and shall be <br /> made available for inspection, upon the request of a representative of the department or a CUPA.The records shall <br /> be retained, after that three-year period, during the course of an unresolved enforcement action or as requested by <br /> the department or CUPA.The records shall include, but not be limited to, all of the following information: <br /> (a)The amount, by volume or weight or both of each material that required special handling. <br /> (b)The method used by the appliance recycler to recycle, dispose of, or otherwise manage each material that <br /> required special handling, including the name and address of the facility to which each material was sent. <br /> (c)The number and types of appliances from which materials that require special handling are removed each year. <br /> (d)The reports required pursuant to subdivision (c)of Section 25211.2. <br /> CORRECTIVE ACTION: Immediately find the missing documents and provide copies to the EHD, if they are <br /> available. Immediately start generating the required documentation, listed above, and keep the records at the <br /> facility for three years. Provide a corrective action statement and supporting documentation to the San Joaquin <br /> County Environmental Health Department(EHD)within 30 days. <br /> This is a repeat violation, Class II. <br /> Overall Inspection Comments: <br /> An inspection checklist was provided to the facility operator on the day of inspection. A complete report was <br /> finished on 7/8/2020 and replaces the initial checklist. <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by August 7, 2020. <br /> The report was emailed to Operations Manager Alex Thomas, at<alex.thomas@simsmm.com>. Starting <br /> September 1, 2018, all in-office CERS help will be provided at EHD hourly rate ($152).
