10.1.1 Preplacement Examination <br /> All employees will receive a preplacement medical examination prior to assignment to field <br /> operations. <br /> 10.1.2 Annual Examination <br /> Each year subsequent to the placement examination, all employees and subcontractors must <br /> ' undergo an annual examination similar in scope to the placement examination. IT employees <br /> hired prior to 1985 are not required to submit to drug screening. Chest X-rays are taken <br /> every third year. The medical and occupational history is updated with each examination. <br /> ' 10.1.3 Exit Examination <br /> IT employees receive an exit examination upon leaving the company if they have not been <br /> examined within the previous six months. The exit examination consists of the annual <br /> ' examination without drug screen. The employee's immediate supervisor is to notify the home <br /> office HS Assistant within a reasonable time before the termination to allow for the necessary <br /> arrangements. <br /> 10.2 First-Aid and Medical Treahnent <br /> All persons on site must report any near-miss incident, accident, injury, or illness to their <br /> ' immediate supervisor or the Field Supervisor. First aid will be provided by the designated <br /> site first aider. Injuries and illnesses requiring medical treatment will be accompanied by an <br /> "Authorization for Treatment" Form. The employee's supervisor or the Field Supervisor will <br /> rcomplete the "Supervisor's Employee Injury Report" and conduct an accident investigation as <br /> soon as emergency conditions no longer exist and first-aid and/or medical treatment has been <br /> rendered. The investigation should follow the Accident/injury Investigation Report. These <br /> two reports must be completed and submitted to the HS Coordinator within 24 hours after the <br /> ' incident. <br /> If fust-aid treatment is required, first aid kits are kept at the CRZ and in all IT vehicles. If <br /> treatment beyond first aid is required, the injured should be transported to the medical facility <br /> listed in Chapter 12.0 of this HASP and the injured's supervisor should immediately contact <br /> EMR at (800) 416-3669. If the injured is not ambulatory or shows any sign of not being in a <br /> comfortable and stable condition for transport, then an ambulance/paramedics should be <br /> MYJo5.09-9a1usr/g4-W& 10-2 <br />
