May 14, 1996 ' Project Name: Former Fabricare Site ' Project Site Location: 711 South San Joaquin Street, Stockton, California Client Contact: Anthony Varni Phone: (510) 886-5000 Project Manager: Joseph Ramage ' The Project Manager :s responsible for all onsite activities. including having all appropriate plans prepared and distributing the pians to IT field personnel and to authorized representatives of each subcontractor employed to perform work on site The Project Manager is also responsible for ensuring that the provisions of the Health and Safety Plan (HSP) are earned out. This includes ensuring that all IT field personnel have received adequate training, that an adequate supply of safety and emergency equipment is available, and that the required safety reports are submitted to ' the regional Health and Safety Officer Designated Site Safety Officer: eGsampe._ T✓ v A 5 t r� E Y b The Site Safety Officer (SSO) is responsible for implementing the site health and safety plan. ' This includes ensuring that field personnel have read and signed the HSP, and that a Tailgate Safety Meeting is conducted on site each day prior to uunating field activities. In addition, the SSO will be responsible for auditing the site operations for compliance with the health and safety ' plan and any applicable federal, state or local requirements. The SSO is required to be on site at all tunes In the SSO's absence, a designated alternate will assume the dunes and ' responsibilities. Work Objectives/Scope: 1 Task 1. Groundwater Sampling Monitor and collect groundwater samples from three on-site groundwater monitoring wells. Task Z. Well Abandonment The three monitoring wells will be abandoned by being filled with cement grout. 1
