• Signing the HSP, acknowledging receipt and understanding of this <br /> document; <br /> • Bringing forth any questions or concerns regarding the content of <br /> the HSP to the HSO, PM, or HSD prior to the start of work; <br /> • Reporting accidents/incidents and the presence of potentially <br /> hazardous working situations to the HSO and PM; and <br /> • Complying with the requests of the appointed HSO. <br /> Training and Documentation Requirements <br /> All ERM and subcontractor personnel working on the project site will <br /> have completed an extensive training course and will have previously <br /> worked at least three days at a hazardous waste site. The training <br /> course must be designed to meet the requirements of 29 CFR 1920.120 <br /> and 8 CCR 5192 (Hazardous Waste Operations and Emergency <br /> Response). The training course must consist of a combination of <br /> 40 hours of classroom and field exercises plus an annual eight-hour <br /> refresher course. <br /> All participants in site field work will be required to show proof of <br /> current training (i.e., initial or refresher training must have taken place <br /> within the past year) prior to the initiation of site activities. Intended <br /> participants who cannot show documentation of current training will <br /> be barred from site activities. <br /> In addition, all workers must show documentation of legal residency <br /> and/or work permit/visa prior to initiation of the project. These <br /> documents may include a Social Security card or any other <br /> appropriately approved work permit. <br /> Medical Surveillance <br /> All on-site personnel and subcontractors will be required to have a <br /> written statement from a licensed physician stating that they have had <br /> a medical examination which meets the requirements of 8 CCR 5192. <br /> This examination must include pulmonary function testing as well as <br /> certification by the physician of the employee's ability to wear a <br /> negative-pressure respirator and perform strenuous work at hazardous <br /> waste sites. If a person sustains an injury or contracts an illness related <br /> to work on site and this injury or illness results in lost work time, <br /> 6-4 <br />
