ATTACHMENT B -2- MONITORING AND REPORTING PROGRAM GENERAL ORDER FOR LAND DISPOSAL OF GROUND WATER FROM CLEANUP OF PETROLEUM FUEL POLLUTION GROUND WATER MONITORING Prior to construction, plans and specification for around water monitoring and/or extraction wells shall be submitted to the Board staff for review and approval . Quarterly reports are required in Section 2652(d) of the Underaround Tank Regulation, Chapter 16, Division 3, Title 23, of the California Code of Regulations. Recommended reporting formats are included in the Tri-Regional Recommendations and Appendix A. Ground water monitoring resuits , volumes of extracted around water and product, and remediation system operational nformation must be included. REPORTING In reporting the monitoring data, the Discharger shall arrange the data in tabular form so that the date, the constituents , and the concentrations are readily discernible. The data shall be summarized in such a manner to illustrate clearly the compliance with waste discharge requirements. 90 days after the issuance of this Order, the discharger shall supply Board staff with a ground water monitoring program that will demonstrate the effectiveness of the ground water treatment system. Annually, by 30 January, the discharger shall submit a report evaluating the effectiveness and progress of the ground water cleanup, including at a minimum: both tabular and graphical summaries of the monitoring data obtained during the previous year; trends in the concentrations of the pollutants in around water monitoring wells; whether the contaminant plume is being captured by the extraction system or is continuing to spread; plans for improvements to the around water monitoring, extraction or treatment system; status of any other '--ieanuD activities such as soil excavation or soli venting systems; and the anticipated date for comoietion of cleanup activities. -he annuai report shall also inciude the reasons for and durations of all interruDtions in the operation of the treatment systems. Upon written request of the Board, the Discharger shall submit a report to the Board by 30 January of each year. The report shall contain both tabular and graphical summaries of the monitoring data obtained during the previous year. :n addition, :he Discharger shall discuss the compliance record and the corrective actions taken or planned which may be needed to bring the discharge nto full compiiance with the waste discharge requirements.
