SAN J O A Q U I N Environmental Health Department <br /> AMENDED <br /> 1/6/2020 <br /> Large Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> PEP BOYS#562 1 1401 S CHEROKEE LN, LODI December 20, 2019 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I.CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 512 CCR 66265.192(h)Failure to obtain a reassessment of a tank system within 5 years or as required. <br /> OBSERVATION: This facility has hazardous waste tanks which have not been reassessed by a Professional <br /> Engineer within the past five years.The tanks are: <br /> 1.a 250-gallon waste oil tank, last assessment completed in February 2012; <br /> 2. a 480-gallon waste oily water tank, last assessment completed in February 2012; and <br /> 3. a 480-gallon waste coolant tank,last assessment completed in February 2012. <br /> REGULATION GUIDANCE: <br /> The assessment specified in subsection(k)of this section shall be obtained prior to placing a new tank system in <br /> service and shall be kept on file at the facility.This assessment shall be reviewed and certified by an independent, <br /> qualified, professional engineer, registered in California, in accordance with section 66270.11(d), attesting that the <br /> tank system has sufficient structural integrity and is acceptable for the transferring, storing and treating of <br /> hazardous waste. The assessment shall be valid for a maximum period of five(5)years or the remaining service life <br /> of the tank system, as stated in the engineer's assessment,whichever is less. <br /> CORRECTIVE ACTION: Provide a corrective action statement and supporting documentation to the San Joaquin <br /> County Environmental Health Department(EHD)within 30 days.
