The following is an itemized list of underground storage tank violations that have not <br /> been addressed for QUIK STOP MARKET #2120* as of May 06 , 2020 . <br /> Open violations from May 02, 2019 inspection <br /> Violation #105 - Failed to have an approved UST monitoring plan . <br /> The monitoring plan is not current and/or not approved by the EHD . The monitoring plans for both tanks do not <br /> include the UDC sensor manufacturer or model . The monitoring plan must be uploaded to the California <br /> Environmental Reporting System ( CERS ) . Immediately log into CERS , make the necessary changes , and submit <br /> for review by the EHD . <br /> ❑ This violation was corrected ❑ This violation will be corrected by ( date) : <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation : <br /> Violation #201 - Failure to maintain monitoring , maintenance , and follow up action records . <br /> Maintenance and monitoring records for the last three years were not found on site . Overfill prevention test results <br /> for 9/26/2018 were not found on site . These records shall be maintained on site for at least three years . Monitoring <br /> records include : ( 1 ) date and time of all monitoring or sampling ; (2 ) monitoring equipment calibration and <br /> maintenance records ; (3 ) results of any visual observations ; (4) results of sample analysis performed a lab or in the <br /> field ; (5 ) logs of all readings of gauges or other monitoring equipment , ground water elevations , or other test results ; <br /> (6) results of inventory readings and reconciliations . Immediately locate and maintain all maintenance and <br /> monitoring records for the last three years on site . <br /> ❑ This violation was corrected ❑ This violation will be corrected by ( date) : <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation : <br /> Violation #202 - Failure to maintain the " Facility Employee Training Certificate" on-site ( after October 1 , <br /> 2018) . <br /> -The Facility Employee Training Certificates contained no CERS ID , no phone number, and the designated operator <br /> expiration date was incorrect. <br /> -The designated employee training conducted on 12/17/ 18 and 11 /21 /18 were on an outdated form . <br /> All site employees must be trained prior to taking responsibility as a facility employee if after October 1 , 2018 , or <br /> within 30 days of hire if prior to October 1 , 2018 . The Facility Employee Training Certificate shall contain accurate <br /> information for all employees trained . Update the Facility Employee Training Certificates to contain the accurate <br /> information and submit copies of the updated forms to the EHD . <br /> Page 1 of 3 <br />
