SITE HEALTH AND SAFETY PLAN <br /> UNION PACIFIC RAILROAD <br /> n ria u <br /> • The Project Manager or Site Safety Officer (SSO) must review the Site Health and Safety Plan (SHSP)with all members of the field <br /> crew,including Antea Group employees and subcontractors,prior to beginning all field work. <br /> • All field team members working in the contamination zone, or who may be exposed during the course of their work, shall have <br /> completed OSHA 40-hour HAZWOPER, annual refresher training(29 CFR 1910.120),CPR and First Aid training, the Union Pacific e- <br /> RailSafe and Contractor On-rail training. Each Antea Group team member must review, sign and date the SHSP and the <br /> Acknowledgement Agreement on page 15. <br /> • Each subcontractor employee and visitor must review the SHSP and sign,date,and describe their affiliation on page 16. <br /> • Signed plan is kept in the field for duration of project and returned to the project file upon completion of field activities. <br /> • SHSP shall be revised or rewritten if site activities are changed significantly, if areas of differing hazard are involved, or as <br /> information about contaminants and hazards changes. Changing conditions may justify either increasing or decreasing SHSP <br /> restrictions and action levels,depending upon the additional information generated. <br /> I.
