Page 2 of 3 <br /> On 07 December 2010,AGE submitted a permit package to abandon five (5)on-site <br /> and two (2) off-site:monitoring wells utilizing over-drill techniques. The permits were <br /> processed-by the.San Joaquin County Environmental.Health Department (EHD) on <br /> 14 December 2010. For your review, I have attached copies of the processed permit <br /> (s), a site plan, and a table that summarized the monitoring well ground water <br /> analytical results. <br /> The off-site wells are identified as wells MW-3.and MW-5. While.marking the wells <br /> for underground service alert, we realized that wells.MW-3 and MW-5 are, <br /> underneath electrical service wires. As a result, I am requesting to abandon wells <br /> MW-3.and MW-5 utilizing pressure grouting techniques as described in the.San <br /> Joaquin County Environmental Health Division Well Standards for abandoning <br /> wells. <br /> As required by EHD.permitting, a health and safety plan will be prepared in <br /> accordance with Occupational Safety and Health Guidance Manual for Hazardous <br /> Waste Site Activities (National Institute for Occupational Safety and Health <br /> Administration, U.S..Coast Guard and U.S..Environmental Protection Agency, 1985).. <br /> Prior to drilling activities, each proposed drilling location will be clearly marked and <br /> an utility clearance obtained through Underground Service Alert. <br /> In accordance with Section 13.12.12. and in preparation for well destruction, wells. <br /> MW-3 &MW-5 will be sounded for total depth and checked for integrity, any <br /> obstruction found in the well will be removed..Over-drilling of the well casing will be <br /> performed to a minimum depth of approximately 3 feet below surface grade(bsg) <br /> utilizing 20-inch hollow.stem augers or equivalent..A six inch riser of casing will be <br /> exposed for sealing purposes.. <br /> The remaining 2-inch diameter well casing will then be completely backfilled with a <br /> Portland type II cement grout mixture. The cement grout mixture will be applied <br /> under pressure (25.psi for at least 10-minutes)..The grout mixture will consist of a <br /> nominal six gallons of water to 94-pound sack portland cement ratio. <br /> Following backfilling procedures, the remaining void space (inside the annular space <br /> left by over-drilling activities) will be completely backfilled with a neat cement to <br /> surface grade..Soil cuttings generated during over-drilling activities and rinseate will <br /> be containerized in properly labeled Department of Transportation(D.O.T.)-approved <br /> 55-gallon drums, and stored on-site in an area lacking public access. <br /> I <br /> 12/23/2010 <br />
