l Well Installation Report MASONITE (former Prem Dor) 433 West Scotts Avenue, Stockton, California 1.0. INTRODUCTION At the request of Mr.Tom Crawshaw of Masonite International,Advanced GeoEnvironmental,Inc. (AGE)has prepared this well installation report for the property located at 433,West Scotts Avenue, Stockton, California(site). The scope of work included 1)the soil sampling of two borings and the installation of two ground water monitoring wells; 2) well development; 3) well-head survey; 4) waste soil disposal and 4) preparation of this report. The site and the surrounding area are illustrated on Figure 1. On-site structures and monitoring well locations are illustrated on Figure 2.Additional site background information and historical analytical soil and ground water sample results are providedin.Appendix A. 2.0. SUMMARY OF SERVICES PROVIDED Two ground water monitoring wells were installed at the site on 19 March 2008 to further delineate the hydrocarbon-impacted ground water at the site. A summary, of the well installation activities include: • The collection of soil sampling from two pilot soil boring for the installation.two ground- water monitoring wells; • Development of the two groundwater monitoring wells;, . • A well-head survey; • Disposal of soil cuttings generated during well installation. 3.0. PROCEDURES The field work was performed in accordance with procedures outlined in the AGE-prepared Additional Site Assessment WorkPlan dated 14 December 2007,which were modified and approved by the San Joaquin Environmental Health Department.(EHD)in their letter dated 07 November 2007 (Appendix B).The work was performed in accordance with the Regional Board and EHD guidelines for the subsurface investigation of underground storage tank(UST) sites.
