l <br /> Well Installation Report <br /> MASONITE (former Prem Dor) <br /> 433 West Scotts Avenue, Stockton, California <br /> 1.0. INTRODUCTION <br /> At the request of Mr.Tom Crawshaw of Masonite International,Advanced GeoEnvironmental,Inc. <br /> (AGE)has prepared this well installation report for the property located at 433,West Scotts Avenue, <br /> Stockton, California(site). The scope of work included 1)the soil sampling of two borings and the <br /> installation of two ground water monitoring wells; 2) well development; 3) well-head survey; <br /> 4) waste soil disposal and 4) preparation of this report. <br /> The site and the surrounding area are illustrated on Figure 1. On-site structures and monitoring well <br /> locations are illustrated on Figure 2.Additional site background information and historical analytical <br /> soil and ground water sample results are providedin.Appendix A. <br /> 2.0. SUMMARY OF SERVICES PROVIDED <br /> Two ground water monitoring wells were installed at the site on 19 March 2008 to further delineate <br /> the hydrocarbon-impacted ground water at the site. A summary, of the well installation activities <br /> include: <br /> • The collection of soil sampling from two pilot soil boring for the installation.two ground- <br /> water monitoring wells; <br /> • Development of the two groundwater monitoring wells;, . <br /> • A well-head survey; <br /> • Disposal of soil cuttings generated during well installation. <br /> 3.0. PROCEDURES <br /> The field work was performed in accordance with procedures outlined in the AGE-prepared <br /> Additional Site Assessment WorkPlan dated 14 December 2007,which were modified and approved <br /> by the San Joaquin Environmental Health Department.(EHD)in their letter dated 07 November 2007 <br /> (Appendix B).The work was performed in accordance with the Regional Board and EHD guidelines <br /> for the subsurface investigation of underground storage tank(UST) sites. <br /> Advanced GeoEnvironmental,Inc. <br />
