The following is an itemized list of hazardous waste violations that have not been addressed for HIGH SPEED TRANSMISSION as of April 09, 2020. Bills of lading or manifests for the management of lead acid batteries for the last three years were not found on site. A copy of each bill of lading must be kept on site for at least three years. Immediately locate a copy of all bills)of lading or manifests for lead acid batteries for the last three years, maintain them on site, and submit copies to the EHD. ❑ This violation was corrected ❑ This violation will be corrected by (date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation#117- Failed to submit a written response within 30 days of receiving an inspection report. An inspection was last done on 7-28-2015 and an inspection report was issued identifying information to be submitted to bring this site into compliance. This information was required to be submitted by 8-27-2017. An incomplete return to compliance statement was received on 9-17-2015. An operator that receives an inspection report shall have 30 days to submit a written response that includes a statement documenting corrective actions taken or proposing corrective actions which will be taken. Ensure that a written response documenting corre tive actions taken or proposed is submitted within 30 days of receiving an inspection report. ❑ This violation was corrected ❑ This violation will be corrected by(date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation #303 -Failed to maintain all communication or alarm systems,spill control, or decontamination equipment.
