The following is an itemized list of hazardous waste violations that have not been <br /> addressed for HIGH SPEED TRANSMISSION as of April 09 , 2020 . <br /> Open violations from July 26 , 2018 inspection <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date) : <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation : <br /> Violation #115 - Failed to retain copy of manifest or bill of lading for spent lead acid batteries for 3 years . <br /> Bills of lading or manifests for the management of lead acid batteries for the last three years were not found on site . <br /> A copy of each bill of lading must be kept on site for at least three years . Immediately locate a copy of all bills of <br /> lading or manifests for lead acid batteries for the last three years , maintain them on site , and submit copies to the <br /> EHD . <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date) . <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation : <br /> Violation #117 - Failed to submit a written response within 30 days of receiving an inspection report. <br /> An inspection was last done on 7-28-2015 and an inspection report was issued identifying information to be <br /> submitted to bring this site into compliance . This information was required to be submitted by 8 -27-2017 . An <br /> incomplete return to compliance statement was received on 9- 17-2015 . An operator that receives an inspection <br /> report shall have 30 days to submit a written response that includes a statement documenting corrective actions <br /> taken or proposing corrective actions which will be taken . Ensure that a written response documenting corrective <br /> actions taken or proposed is submitted within 30 days of receiving an inspection report. <br /> ❑ This violation was corrected ❑ This violation will be corrected by (date) : <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation : <br /> Violation #303 - Failed to maintain all communication or alarm systems , spill control , or decontamination <br /> equipment. <br /> Afire extinguiser on the wall near the front entrance was observed not charged . All facility communications or alarm <br /> systems , fire protection equipment, spill control equipment, and decontamination equipment, where required , shall <br /> be tested and maintained as necessary to assure its proper operation in time of emergency.
