A J OA U I Environmental Health Department <br /> COUNTY <br /> Small Quantity Hazardous Waste Generator Inspection Report <br /> Facility Name: Facility Address: Date: <br /> JOES TRAVEL PLAZA 1 15600 S HARLAN RD, LATHROP February 13, 2020 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 106 CCR 66262.34(d)(2) Failed to train employees on waste handling and emergency procedures. <br /> OBSERVATION: At the time of inspection, it could not be demonstrated that employees who handle hazardous <br /> waste were properly trained. <br /> REGULATION GUIDANCE: The business owner must ensure that all employees are thoroughly familiar with proper <br /> waste handling and emergency procedures, relevant to their responsibilities during normal facility operations and <br /> emergencies. <br /> CORRECTIVE ACTION: Immediately train the employees. Provide proof of training within 30 days to the San <br /> Joaquin County Environmental Health Department(EHD)for employees whose responsibilities include hazardous <br /> waste. <br /> This is a minor violation. <br /> 109 CCR 66262.23(a)(4) Failed to send generator manifest copies to DTSC within 30 days. <br /> OBSERVATION: Generator copies of Uniform Hazardous Waste Manifest (UHWM) numbers <br /> -019676625 dated 10/25/2019 <br /> -020403232 dated 07/12/2019 <br /> were not sent to the California Department of Toxic Substances Control (DTSC)within 30 days of hazardous waste <br /> shipment. No information found on HWTS ( hazardous waste tracking system), the generator stated that they did <br /> not mail the manifest they relied on the transporter to mail them. <br /> REGULATION GUIDANCE: Hazardous waste generators are required to send a copy of the generator signed <br /> UHWM to DTSC by mail. The only exception is when an UHWM starts and finishes as an electronic manifest in the <br /> e-Manifest System. The generator shall within 30 days of each shipment of hazardous waste submit to DTSC a <br /> legible copy of each manifest used. The mailing address is: DTSC Generator Manifests, Department of Toxic <br /> Substances Control, P.O. Box 400, Sacramento, CA 95812-0400. <br /> CORRECTIVE ACTION: Immediately mail a copy of the listed UHWMs and ensure that copies of all generator <br /> signed UHWMs are sent to DTSC within 30 days of shipping the hazardous waste.
