(AGE) <br /> is providing the following information regarding encroachment permit application at the referenced <br /> site: <br /> 1. This letter should be considered formal written acknowledgement that the permit is <br /> revokable and that the City may revoke the permit at any time. <br /> 2. A copy of the San Joaquin County Public Health Services - Environmental Health Division <br /> (PHS-EHD)permit will be mailed as soon as it is approved. <br /> 3. AGE has previously provided the City of Lathrop with a check for$120.00 for encroachment <br /> permit fees. <br /> 4. We are presently working with Mr. Langston to provide the required $5,000.00 bond. <br /> 5. A check for the inspection fee for $300.00 is attached.. <br /> 6. A copy of the work plan is attached as well as a $150.00 Plan Check Fee. <br /> 7. A copy of an Engineer's Cost Estimate is attached, a 20% faithful performance bond will be <br /> submitted with the previously mentioned bond. <br /> 8. A traffic control plan is not applicable due to the location of the proposed monitoring wells. <br /> 9. USA will be notified prior to monitoring well construction. The city will have final decision <br /> on location of the wells. <br /> 10.
