Seventh St. RO#: 00316 <br /> Lathrop, CA., 95330 SWRCB-CUF#: 00367 <br /> On October 3, 2005, San Joaquin County Environmental Health Department (EHD) <br /> approved the Additional Site Assessment Work Plan submitted by your consultant <br /> Advanced GeoEnvironmental (AGE). <br /> The work plan proposed installing at least two soil borings offsite to the east to <br /> investigate the lateral limit of the ground water plume associated with the unauthorized <br /> release from the former underground storage tanks (UST) located at the above <br /> referenced site. In the approval letter EHD established dates when specific tasks of the <br /> work plan were to be completed. The completion dates established were for drilling two <br /> soil borings, installation of additional borings or a monitoring well based on the data <br /> from the two borings, and submittal of a Site Conceptual Model report (SCM). <br /> As of the date of this letter none of the tasks approved in the work plan have been <br /> enacted. <br /> You are required to investigate the nature and extent of the contamination associated <br /> with the unauthorized release from your USTs. By not enacting the approved work plan <br /> you are not complying with directives and requirements issued by EHD. <br /> You are hereby directed to enact the tasks contained in the September 30, 2005 <br /> work plan approved by EHD. Fieldwork to install the soil borings is to be <br /> completed by March 17, 2006. A report evaluating the need for additional <br /> monitoring wells in this area is due at EHD by April 21, 2006. If the monitoring <br /> wells are needed, they are to be installed by May 5, 2006 and the SCM is due at <br /> EHD by June 30, 2006. If it has been determined the monitoring wells are not <br /> needed, the SCM is due at EHD by May 17, 2006. <br /> Please submit a Well Permit Application Form and $368 fee immediately. <br />
