Eyrene Fisher. <br /> STATUS: Approved, with conditions noted below. <br /> ADEQUATE AND NECESSARY ELEMENTS OF THE WORK PLAN [H&S 25296 10 subd(c)(3)1: <br /> • Install at least two (2), possibly three (3) offsite soil borings to 50-feet deep to <br /> investigate the lateral extent of the petroleum plume and, <br /> • Based on presence of offsite contamination, possibly install one,first ground water <br /> monitoring well to 28 feet below ground surface to continue lateral ground water <br /> plume investigations and, <br /> • Submit a Report of Findings to EHD. <br /> COMMENTS AND CONDITIONS: <br /> • In order to avoid any additional delays with completing the investigative phase for <br /> this UST site, EHD directs the following schedule: <br /> o Install at least two soil borings by October 31. 2005. <br /> o Based on soil and ground water analytical data from the two soil borings, <br /> evaluate the need for the addition boring and the monitoring well and if <br /> needed, install them by November 30,2005. <br /> o If no monitoring well is installed,the report of findings is due at EHD by <br /> December 6. 2005 in the format of a Site Conceptual Model (SCM). <br /> o If the monitoring well is installed,the report of findings (SCM)for the soil <br /> borings and the monitoring well soil and ground water data from it, is to be <br /> submitted to EHD no later than January 30. 2006. <br /> • The SCM shall include discussions and evaluations of residual mass(including <br /> lead), migration concerns, additional investigative requirements, if any, and <br /> recommendations for either remedial action or no further action. <br /> REASONS FOR DISAPPROVAL OF THE WORK PLAN [H&S 25296.10 subd(c)(3)1: <br /> • none <br /> DATED: October 3, 2005 BY <br /> Michael Infurna, -e r REHS (209) 468-3454 <br /> [X] Faxed to consultant on: October 3. 2005 i <br />
