Additionally, a <br /> proposal to do what is required is unneeded. <br /> The State Water Resources Control Board (SWRCB) Clean Up Fund (CUF) will reimburse <br /> investigative and remedial costs incurred when directed or n�quiredbyPHS-EHD. You are <br /> hereby directed to monitor, sample, and report on the groundwater contaminant <br /> concentrations and site conditions on a quarterly basis until directed otherwise by <br /> PHS-EHD. <br /> The Quarterly Monitoring Reports (QMRs) are to be submitted to PHS-EHD and the <br /> Central Valley Regional Water Quality Control Board (CVRWQB) by the 15`s of the month <br /> following the quarter's end. The QMRs are to include all of the field data collected as well as <br /> the California Department of Health Services certified laboratory quality assurance <br /> information. The QMR is to include professional conclusions/interpretations of the data <br /> and conditions of the site as evaluated by a California Registered Geologist or Engineer. <br /> Recommendations are to be included in every QMR and are to provide PHS-EHD with a <br /> glance of the potential future plans for the site. <br /> Sites that do not submit complete QMRs regularly will not be in compliance and jeopardize <br /> their ability to become or remain eligible to receive reimbursement from the SWRCB-CL)F. <br /> A Division of San Joaquin County Health Care Services <br />
