Governor <br /> For a list ofsimple ways you can reduce demand and cul your energy costs,see our websile at ulrw.swrcb.ca.gov. <br /> MAY 15 ;k1rn <br /> Richard Belthoff, Jr. a <br /> Compass Group USA, Inc. <br /> 2400 Yorkmont Rd <br /> Charlotte, NC 28217 <br /> UNDERGROUND STORAGE TANK CLEANUP FUND (FUND), REQUEST FOR FURTHER <br /> DOCUMENTATION DURING IMTIAL REVIEW: CLAIM NUMBER-016536; FOR SITE <br /> ADDRESS: 1500 SHAW RD, STOCKTON <br /> After reviewing your claim application to the Cleanup Fund,we find that the following additional <br /> information is needed to determine your eligibility for placement on the Priority List: <br /> A copy of the permit to own or operate the UST from the local implementing agency dated between <br /> January 1, 1984 and January 1, 1990 (pursuant to Chapter 6.7 of the Health and Safety Code). <br /> If you were not subject to the permit requirement, submit documentation to confirm this claim. Situations <br /> where the permit was not required by January 1, 1990, can include: a)you removed all USTs prior to <br /> January 1, 1990; and not replaced; b) you decommissioned all USTs pursuant to the direction of the <br /> regulatory agency prior to January 1, 1984; c)you sold the property and tanks by January 1, 1990. <br /> If you were subject to the permit requirement but failed to comply by January 1, 1990,you can request <br /> the State Board to waive the requirement as a condition for eligibility. To request a waiver, complete the <br /> enclosed"Permit Waiver Request" form and return with any additional information requested below. <br /> Note: If you are granted a Permit Waiver,you must demonstrate financial responsibility in the <br /> amount of at least$20,000. <br /> The Petroleum Underground Storage Tank Cleanup Fund Claim Application Forms and Instructions <br /> (enclosed) outlines the claims process. First is submittal of a complete claim application so that the Fund <br /> may make a determination of eligibility. Second is acceptance of the claim and placement on the <br /> appropriate priority list for funding. Third is issuance of a Letter of Commitment(LOC) obligating funds <br /> to reimburse eligible costs. Fourth is the mailing of the LOC including the Reimbursement Request <br /> Instructions providing the forms and details on submitting a reimbursement request.. Fifth is submittal of <br /> the reimbursement request and its supporting documentation. <br /> Review of the file indicates that a request for reimbursement of funds was submitted prematurely with the <br /> claim application. After the claim application is accepted and a LOC is issued, instructions and forms <br /> needed to support the reimbursement requests will be mailed with your copy of the LOC. Note: Priority <br /> Class D claims are the lowest priority and will not be funded for several years. <br /> I am returning the spreadsheet, invoices,bid summary sheets, and bids that you submitted with the claim <br /> application.
