(AESI) on January 16,2001 and has the following comments.
On September 18,2000,PHS-EHD responded to an identical proposal by AESI dated August 31,
2000. PHS-EHD maintains our position on requests for approval on rapuired work. As stated in our
response dated September 18, 2000,"ytv are hereby directed to nvnitor, sanpk and report on the gnwxivater
conianimrit mnxr&ratiwas and site condeim on uyjna basis med dimmed arse by PHS-EHD". PHS-
EHD has issued no changes in this requirement and you are still required to complywith the
directive issued to you on September 18,2000. Your January 12,2001 request for approval to
sample the monitoring wells associated with the underground storage tank(UST) for an additional
year after PHS-EHD has directed you to do this previously is redundant and unneeded.
If you are experiencing difficulty with obtaining reimbursement for this requirement from the State
Water Resources Control Board (SWRCB) UST Cleanup Fund (fund), please contact me by phone
so that I may assist you with obtaining this cost from the fund. If there is no difficulty with the fund
at this time, you should contact Wennilyn Fua at (916) 341-5674 to ensure that your standard claim
information reflects that quarterly monitoring, sampling and reporting associated with this site are
required byPHS-EHD.
PHS-EHD has evaluated this site recently and has determined that the lateral and vertical extent of
the petroleum plume has been mostly delineated. The investigative phase of the project appears to
be over or is very close. The Problem Assessment Report(PAR) should now be submitted to
PHS-EHD. Please refer to the Tri-Regional Guidelines,Appendix A,for the minimum
requirements that must be included in the PAR If you determine that more site work is needed to
complete the PAR, a work plan that proposes to gather that information is to be submitted to PHS-
EHD by Manch 1, 2001. If there is adequate information to complete the PAR now,you are hereby
directed to submit the Problem Assessment Report to PHS-EHD before July 16,2001. The PAR is
to include the quarterly groundwater data from both the first and second quarters of 2001.
A Division of San Joaquin County Health Care Services
