RECE ED <br /> MAY 0 3 204 <br /> April 30, 2004 �1 YCES� <br /> ENVIRONMENT HEALTH <br /> PERMIT/SERVICES <br /> Mr Michael J Infurria, Jr , Senior REHS <br /> San Joaquin County PHS <br /> Environmental Health Division <br /> 304 East Weber Avenue, Third Floor <br /> Stockton, California 95202 <br /> RE Remediation System Installation <br /> Former Canteen Facility, 1500 Shaw Road, Stockton, California <br /> Site Code 1727 <br /> AES Project No.: 145-94009-06 <br /> Dear Mr Infurna <br /> • Applied Earth Sciences, Inc (AES) has completed installation of the sparge/vent remediation <br /> system at the above referenced site The remediation system consists of thirteen air sparge wells <br /> (AS-1 through AS-13), five vapor extraction wells (VE-1 through VE-5), and an internal <br /> combustion engine (ICE) unit for air emission treatment (Figures 1 and 2) The ICE unit is set <br /> on a concrete pad and is enclosed in a secure, fenced compound A propane tank which supplies <br /> supplemental fuel to the ICE unit is also enclosed inside the fenced compound The system was <br /> placed into operation on March 24, 2004 <br /> The remediation system was installed and is operated in accordance with the following permits <br /> • Well Permit Application Form, San Joaquin County EHD, February 9, 2004 <br /> • Authority To Construct, San Joaquin Valley APCD, March 4, 2004 <br /> • Permit to Operate, San Joaquin Valley APCD, (pending) <br /> Copies of the current permits are attached The Permit to Operate will be forwarded when <br /> issued <br /> Remediation System Installation <br /> Well Installation <br /> Thirteen air sparge and three vapor extraction wells were installed during the period from <br /> February 23 through March 5, 2004 The wells were installed under the supervision of a <br /> Registered Geologist in the State of California Cascade Drilling, Inc was subcontracted by <br /> 9009 Mountain Ridge Dr, Suite 110 • Austin, Texas 78759 • Tel (512) 990-7467 • Fax (512) 990-7399 <br /> www appliedearth com <br />
