SAN J O A Q U I N Environmental Health Department -- C 0 u N T Y - AMENDED 1 /9/2020 Large Quantity Hazardous Waste Generator Inspection Report Facility Name: Facility Address: Date: PROFLEET TRUCK LUBE 15453 N THORNTON RD , LODI January 08 , 2020 SUMMARY OF VIOLATIONS (CLASS I, CLASS II, or MINOR - Notice to Comply) Item # Remarks ( 1 ) the job title for each position at the facility related to hazardous waste management, and the name of the employee filling each job ; (2 ) a written job description for each position listed under paragraph (d )( 1 ) of this section . This description may be consistent in its degree of specificity with descriptions for other similar positions in the same company location or bargaining unit , but shall include the requisite skill , education , or other qualifications , and duties of employees assigned to each position ; (3) a written description , including a syllabus and/or outline , of the type and amount of both introductory and continuing training that will be given to each person filling a position listed under paragraph (d )( 1 ) of this section ; (4 ) employee-signed or -certified records that document that the training required under subsections (a ) , (b ) , and (c) of this section has been given to , and completed by, each employee . (e) The owner or operator shall maintain training records on current personnel until closure of the facility and training records on former employees for at least three years from the date the employee last worked at the facility. Personnel training records may accompany personnel transferred within the same company. CORRECTIVE ACTION : Immediately provide proof that all employees who work with hazardous waste have been properly trained in compliance with the time frames and training materials required by this section . In addition , provide a written description showing the type and amount of both introductory and continuing training that is required for employees . Provide training documentation to the San Joaquin County Environmental Health Department (EHD ) within 30 days . This is a Class II violation . 124 HSC 25404 (e )(4 ) Failed to report program data in the California Environmental Reporting System (CERS ) . OBSERVATION : The business owner changed to Lubezone Inc. in May 2019 and a submittal showing the business's hazardous waste activities has not been made on the California Electronic Reporting System (CERS ) website . REGULATION GUIDANCE : Each regulated business shall report program data electronically on the CERS website . CORRECTIVE ACTION : Immediately report your business activities including the hazardous waste program on the CERS website . Provide a corrective action statement to the San Joaquin County Environmental Health Department within 30 days . This is a Class If violation .
