(VERTEX) is pleased to present this Work Plan associated with the <br /> performance of a Phase II Limited Subsurface Investigation (Phase II) at the Lodi Memorial Park <br /> property located at 5750 East Pine Street in Lodi, California(the site). <br /> PROJECT WORK SCOPE <br /> Utility Clearance and Preparation of Health and Safety Plan <br /> Health &Safety Plan Preparation <br /> A Health& Safety Plan (HASP) will be prepared to guide the conduct of the work in the <br /> event that petroleum hydrocarbons and/or hazardous substances are encountered during <br /> the performance of the field activities. It is the policy of VERTEX to prepare a HASP for <br /> all sites where petroleum hydrocarbons and/or hazardous substances may be present. The <br /> purpose of the HASP is to minimize the likelihood of exposure of VERTEX employees to <br /> hazardous concentrations of chemicals during field activities, minimize impacts to the <br /> environment, and provide guidelines for subcontractors. <br /> Utility Clearance <br /> VERTEX and/or VERTEX's contractor will contact all appropriate local and state utility <br /> clearance companies, prior to initiating the drilling activities. As necessary, VERTEX <br /> will also coordinate with the property owners and/or site contact, or obtain existing utility <br /> plans, in order to attempt to clear all boring locations as being free of underground utilities. <br /> As such, VERTEX and/or VERTEX's contractor will not be responsible for any damage to <br /> underground utilities,lines,or conduits as a result of drilling activities. <br /> TMZTEZ <br /> Environmental Services, Inc. <br />
