Sonora St. <br /> Stockton, CA <br /> Public Health Services, Environmental Health Division (PHS/EHD) , <br /> has reviewed the Applied Geosystems Monitoring Proposal dated April <br /> 5, 1991, and has the following comments: <br /> 1. PHS/EHD does not comment on the costs or budgets of the <br /> monitoring process, however, pursuant to Section 2652 (d) <br /> of the Underground Storage Tank Regulations (California <br /> Code of Regulations, Title 23, Subchapter 16) a quarterly <br /> monitoring report shall be submitted to Public Health <br /> Services, Environmental Health Division and Central <br /> Valley Regional Water Quality Control Board (CVRWQCB) <br /> until the investigation and cleanup of the site is deemed <br /> adequate. The report is to show the: <br /> -status of investigation and cleanup activities including <br /> the results of all investigations completed to date; <br /> -proposed continuing or next phase of investigation; <br /> -method of cleanup proposed or implemented, schedules for <br /> the completion of the investigation of the site and <br /> remediation; <br /> -method and location of disposal of the released <br /> hazardous substance and any contaminated soil, <br /> groundwater or surface water; <br /> -manifest required for transport of hazardous substances; <br /> -monitoring well data including groundwater elevation, <br /> pH, conductivity, gradient; <br /> -analytical results of groundwater or soil sampling; and <br /> -site map showing the "zero line" of contamination, if <br /> known, and changes in the above measurements over the <br /> last quarter. <br /> A Division of San Joaquin County Health Care Servim <br />
