%Now' <br /> WELL DESTRUCTION WORK PLAN <br /> University Park <br /> 1252 North Stanislaus Street <br /> Stockton, CA 95202 <br /> September 17,2013 <br /> 1.0 INTRODUCTION <br /> This work plan was prepared by Condor Earth Technologies, Inc. (Condor) to describe the proposed <br /> destruction of nine groundwater monitoring wells at the Stockton Developmental Center,University Park, <br /> 1252 North Stanislaus Street(site) in Stockton, California(Figures 1 and 2,Appendix A).The purpose of <br /> the work is to properly destroy all of the monitoring wells prior to final closure of the site.In a letter dated <br /> June 4, 2013, Adrienne Ellsaesser of the San Joaquin County Environmental Health Department <br /> (SJCEHD) indicated that the investigation and remediation of the site had been completed and that their <br /> agency will request destruction of the monitoring wells that are on the site. In a letter dated June 10, 2013, <br /> the Central Valley Water Quality Control Board (CVWQCB) indicated that they concurred with the <br /> SJCEHD letter dated June 4, 2013. The preparation of a work plan for destruction of all wells associated <br /> with the site is part of the site closure process. Condor prepared this work plan at the request of CSU <br /> Contact, Steven Lohr,Chief of Land Use Planning and Environmental Review of the Stockton Center Site <br /> Authority(SCSA)in response to the SJCEHD and CVWQCB letters. <br /> All work conducted by Condor will be under the supervision of a California Professional Geologist. This <br /> work plan includes a description of the field and reporting procedures proposed to conduct the work, and <br /> a project schedule. <br /> 2.0 SITE PREPARATION <br /> A well destruction permit will be obtained from the SJCEHD. Prior to initiating the fieldwork, the well <br /> locations will be marked with white paint and Underground Service Alert (USA) will be notified at least <br /> 48 hours in advance of the fieldwork to locate and identify underground utilities in the vicinity of the <br /> work area. Prior to commencement of fieldwork, a tailgate safety meeting will be held and a site-specific <br /> Health and Safety Plan will be discussed with,and presented to, all field personnel for their signatures. <br /> 3.0 DESTRUCTION OF MONITORING WELLS <br /> A California C-57 licensed driller will perform the destruction of the monitoring wells in accordance with <br /> State of California Department of Water Resources(DWR)and SJCEHD requirements.Because the wells <br /> are located in an area of known petroleum hydrocarbon contamination, and all wells except MW-6 and <br /> MW-9 have had reportable concentrations of hydrocarbon constituents in the past, the SJCEHD well <br /> destruction standards require complete over-drilling of seven of the nine monitoring wells. A Condor <br /> Professional Geologist will observe the well destructions. Monitoring well construction details are shown <br /> in Table A, Appendix A. <br /> Each monitoring well casing will be sounded for water level and total depth prior to destruction. The <br /> monitoring wells, with the exception of MW-6 and MW-9, will be destroyed by over-drilling from top to <br /> bottom so that all material within the original borehole(including the well casing, screen, filter pack, and <br /> annular seal) is removed, and the hole will be filled completely with neat-cement grout slurry.
