Brian A. Waite Page 2 of 2
242 N. Sutter Street October 29, 2013
Stockton, California 95202
limits may be destroyed by pressure grouting from the bottom upward, while leaving the
well casing in place. The top three feet or more of the well shall be drilled out with an
auger that is at least one foot larger in diameter than the auger originally utilized to install
the well, and the soil around the cut-off casing removed to a depth of an additional six
inches below the new `top' of the cut-off casing. The grout sealant shall spill out of the
top of the well casing to form a mushroom cap seal, with at least 12 inches of neat
cement grout poured 6 inches above and 6 inches below the top of the cut-off casing.
• All groundwater displaced to the surface during grout installation shall be pumped out of
the excavation to allow for the EHD inspection and verification that the well has been
properly destroyed. Contaminated groundwater pumped out of the excavation shall be
containerized for proper disposal.
After the EHD has approved the well destruction scope of work proposal, the contactor may
submit an EHD Well and Boring Permit Application form, Master File Record form, and the
appropriate fees. The permit fee for the destruction of each well is $125.00, along with a $375.00
Inspection Activity fee per parcel. A review of the site records indicates that eight wells currently
are installed. A fee of $1,375.00 (8 wells x $125.00 + $375.00) will be required for the well
destruction permit. The EHD will approve and issue the well destruction permit only if the
destruction methods noted on the permit meet with the requirements specified within this letter.
Please be aware that you may be billed at a rate of $125 per hour for any further enforcement
activity required by the EHD to obtain the documents or actions the EHD has directed within this
letter.
Please provide the EHD certification in writing that all record owners of fee title to the above-
referenced site have been notified of your request for closure. This is in accordance with Section
25297.15 of the California Health and Safety Code. Submit a complete list of record owners of fee
title and their mailing addresses to the EHD. If you are the sole landowner, provide a statement to
this effect to the EHD. A form has been enclosed for this purpose.
If you have any questions please contact Nuel Henderson at (209) 468-3436, or by email at
nhenderson@sjcehd.com.
Sincerely,
Nuel C. Henderson, Jr., PG Adrienne Ellsaesser, REHS
Engineering Geologist Program Coordinator
c: James Barton, CVRWQCB, 11020 Sun Center Drive Ste. 200 Rancho Cordova, CA 95670
Justin Sobieraj, ARCADIS U.S., Inc., 2000 Powell St., 7th Floor, Emeryville, CA 94608
