The Discharger shall comply with Monitoring and Reporting Program No. , which is part of this Order, and any revisions thereto as ordered by the Executive Officer." page 9-5 Add the attached Monitoring and Reporting Program: Item 9.c.- BEN'S TRUCK AND EQUIPMENT, INC., AND JERRY ARBINI, SOIL TREATMENT FACILITY, Tehama County page 9-18 Discharge Specification No. 8.5. - change "l May" in the last sentence to "15 April". page 9-21 Provision No. C.2. - at the end of the first sentence add the following phrase: and shall certify that the waste is nonhazardous in accordance with the California Health and Safety Code, Section 663O5(b)." pages 9-26 MONITORING AND REPORTING PROGRAM - several changes were made to thru 9-28 the first three pages of the Monitoring and Reporting Program as a result of comments from various agencies reviewing the permit. See attached pages for those changes. Deletions are indicated by strikeouts and additions are indicated by bold italics. Item 9.e. - KOA CAMPGROUND, TROTTER ENTERPRISES, Calaveras County page 9-48 Change the name of the Discharger in the title from "KOA Campground, Trotter Enterprises", to "Trotter Investments, Inc. , dba Angels Camp KOA, Trotter Enterprises", and wherever else it occurs in the Order. page 9-48 Finding 1 : Should read, Trotter Enterprises submitted a Report of Waste Discharge, dated 12 March 1991, and site evaluation reports, dated 28 March 1991 and November 1991. The operator is Trotter Investments, Inc., and the property (Assessor's Parcel Nos. 64-007-10, 47 and 64-006-11) is owned by Trotter Enterprises. Trotter Enterprises is a partnership of the Living Trust of Jean Trotter and the Testamentary Trust of Donald Trotter. Jean Trotter is the trustee for both trusts. Hereafter, the term Discharger refers to both Trotter Enterprises and Trotter Investments, Inc. ."
