Please include claimant's tax identification number. Note that claimant's tax identification number must correspond with the claimant name(e.g. individual-social security number; corporation,partnership,estate, trust-federal tax identification number). If Mabel E.Keir is the same entity(as listed on the federal tax returns you submitted),then you need to add the E.to Claimant Name on page one. • On page two of your claim application under Description of UST Use,you checked the box for Other. Please describe Other to show what USTs were used for. • On pages five(5)and six(6)of your claim application you applied for Priority Class"C". However, from documentation submitted separately on your behalf by Advanced GeoEnvironmental,Inc. (AGE),it appears that you are applying for Priority Class"B". If this is correct,then please revise pages five and six to reflect Priority Class`B". • Additionally received from AGE was a Request for Assignment of Claim to Priority Class B form that was incomplete. Priority Class"B" small business is for those businesses that are independently owned and operated,not dominant in its field of operation and employs 100 or less employees,including all affiliates, and averages annual gross receipts of$10 million dollars or less over the previous three years. If a business is a manufacturer,there is no revenue test. However,the manufacturer must also employ 100 or less employees including affiliates. If you believe that you qualify,you may request the Fund review your claim to determine eligibility for Priority Class"B". In order to determine eligibility,the following documents must be submitted and reviewed: • Complete the Enclosed form "Request for Assignment of Claim to Priority Class B".
