Box 6569 <br /> Modesto, California 95350 Stockton, California 95206 <br /> Subject: Lopez Properties <br /> 1601 Turnpike Road <br /> Stockton, California 95206 <br /> The San Joaquin County Environmental Health Department (EHD) has reviewed Additional <br /> Site Assessment Work Plan — Monitoring Well Installation dated September 14, 2009, <br /> prepared and submitted by Advanced GeoEnvironmental, Inc. (AGE) for the above-referenced <br /> site on your behalf. In the work plan, AGE proposes to install one monitoring well (MW-12), <br /> screened between approximately fifty-five and sixty-five feet below surface grade (bsg), <br /> approximately one-hund{ed-thirty feet east/southeast of monitoring well MW-11. AGE <br /> proposes to install MW-1? at the property with assessor's parcel number (APN) 165-380-07. <br /> If the property owner will not grant access to this property, then AGE proposes to install the <br /> well in the City of Stockton right-of-way on the east side of French Camp/Turnpike Road, <br /> adjacent to APN 165-380i07. <br /> The EHD approves the installation of MW-12 as proposed by AGE in the work plan. Soil <br /> samples will be collected at five-foot intervals beginning at approximately fifty feet bsg for <br /> lithologic evaluation and screened in the field for volatile organics using an organic vapor <br /> meter with a photo ionization detector; selected Soil samples will be analyzed for total <br /> petroleum hydrocarbons quantified as gasoline (TPH-g); benzene, toluene, ethyl benzene, <br /> total xylenes (BTEX); di isopropyl ether (DIPE), ethyl tertiary-butyl ether (ETBE), methyl <br /> tertiary-butyl ether (MTBE), tertiary-amyl methyl ether (TAME), tertiary-butyl alcohol (TBA), <br /> methanol, ethanol, 1,2-dibromoethane (EDB), and 1,2-dichloroethane (1,2-DCA). Following <br /> the installation of MW-12; groundwater samples will be collected quarterly for one year and <br /> analyzed for the same constituents listed for soil analysis. <br /> Submit a copy of the access agreement or an encroachment permit issued by the City of <br /> Stockton; a boring permit application and permit/inspection fee of $434; notify the EHD of <br /> scheduled fieldwork at least seventy-two hours prior to the installation, and complete the well <br /> installation by December 23, 2009.
