A nearly empty container of Palmolive Ultra Dish Liquid soap, without any spill clean-up material to <br /> account for the missing liquid.The container was not marked with a "store-use" code. David did not <br /> know what happened to the liquid that was once in the container. <br /> c. When asked how cardboard boxes, which become contaminated from leakage or spillage of a chemical <br /> product, are managed, David stated the cardboard is recycled. <br /> Dollar General#15928: <br /> 1. Violation 106—Submit a corrective action statement that specifically addresses each of the following <br /> issues/discrepancies observed at the time of inspection: <br /> a. An expired box of DG Health Coated Nicotine Gum, with an expiration date of 02/2019, was found for <br /> sale on a shelf in the facility. Sue removed the expired nicotine gum from the sales shelf and placed it <br /> into one of two open unlabeled baskets behind the front register. Sue stated that damaged items are <br /> placed into these two baskets behind the register and are processed once a week on Tuesday. Sue <br /> indicated that once a damaged item is processed it would be placed in a HW tote if needed. <br /> b. A Satin Crush Lipstick was found in one of the open unlabeled basket behind the front register. Sue <br /> stated the basket is used to hold damaged items until they are processed every Tuesday. Sue stated that <br /> because the lipstick appeared to be opened,the lipstick would be damaged out and eventually placed in <br /> a HW tote. <br /> Dollar General#13585: <br /> 1. Violation 103-Submit documentation that demonstrates to employees which hazardous waste category(i.e. <br /> Toxic, Flammable, Alkaline,Acid, Oxidizer, etc.) the following wastes belong to: <br /> a. Seven to eight wax candles, one brand which was called True Living in Vanilla Sugar scent(found in a <br /> Flammable Hazardous Waste tote) <br /> b. An unbranded wax candle (found in a Toxic Hazardous Waste tote) <br /> c. Approximately five individually packaged Bowl Fresh (toilet) Bowl Cleaner and Air Fresheners <br /> 2. Violation 106— <br /> a. Submit a corrective action statement that specifically addresses each of the following <br /> issues/discrepancies observed at the time of inspection: <br /> i. Julie indicated that when the scanner does not specify which HW tote an item belongs in, <br /> employees will read the item's label and ingredients, as well as the Dollar General (DG) HW <br /> guidance poster and documents, which provide product examples and key words for each of the <br /> HW categories, to determine which HW tote an item belongs in (see photos IMG 8049, 8051-7). <br /> When asked what employees do if they are still unsure about which HW tote a waste item <br /> belongs in,Julie stated she will place the item into the Toxic HW tote.Julie did not know who to <br /> call in case she had questions about an item's hazardous waste determination.Additionally,Julie <br /> was not aware of Dollar General's (DG) Respond ticketing system to submit a notification to DG <br /> Corporate regarding the scanning system not providing a HW category for a particular waste <br /> item. <br /> b. Submit proof of completion of 2019 California Environmental Compliance for Sales Associates training <br /> for Andrea Heaton and Tatianna Roark. <br /> c. Submit proof of completion of 2019 California Environmental Compliance for Store Managers training <br /> for John Kendall. <br /> Dollar General#13568: <br /> 1.
