The following is an itemized list of hazardous waste violations that have not been <br /> addressed for Dollar General #13568 as of May 29, 2020. <br /> Open violations from July 31, 2019 inspection <br /> Violation#106-Failed to train employees on waste handling and emergency procedures. <br /> Based on the types of violations observed,the facility employees are not trained on proper hazardous waste(HW) <br /> handling or the training provided has been inadequate. <br /> 1. When asked how employees determine which of the differently categorized HW totes a waste item belongs in, <br /> Jackie Nash,Assistant Store Manager, said each item is scanned using a hand-held scanner,which will <br /> sometimes state which HW tote the item belongs in. Jackie indicated that when the scanner does not specify which <br /> HW tote an item belongs in, employees will read the item's label and ingredients, as well as the PSC HW guidance <br /> poster and documents,which provide product examples and key words for each of the HW categories, to <br /> determine which HW tote an item belongs in (IMG 7952-3, 7988-9).When asked what employees do if they are <br /> still unsure about which HW tote a waste item belongs in, Jackie did not seem aware to call the store maintenance <br /> hotline phone number, listed at the bottom of the PSC HW guidance documents.Additionally, Jackie and Steve <br /> did not seem aware of Dollar General's Respond ticketing system to submit a notification to DG Corporate <br /> regarding the scanning system not providing a HW category for a particular waste item. <br /> 2. The waste items listed below were found bagged and inside a tote labeled Toxic HW(IMG 7937-46, 7988): <br /> -A tube of all purpose Krazy Glue. Steve Bohnak, District Manager, stated that he believed many adhesives or <br /> glues were flammable.A tube of The Original Super Glue was found in the HW tote for Flammable HW(IMG 7964, <br /> 7967-8).Also the PSC HW guidance document for Flammable HW pictured a tube of adhesive called Goop. <br /> -A bag containing seven containers of Silk Luxury Collection Moisturizing Body Wash,two of which were missing <br /> their caps and seemed to be empty. There was no spill clean-up material found to account for the liquid missing <br /> from the containers, and Jackie Nash,Assistant Store Manager, did not know what happened to the missing liquid. <br /> 3. The waste items listed below were found bagged and inside a tote labeled Oxidizer HW(IMG 7959-61): <br /> -A leaking container of Arm &Hammer Laundry Detergent Plus Oxi Clean with Fade Defense Sparkling Waters. <br /> According to the PSC HW guidance poster and documents, laundry detergent is to be placed in a Toxic HW tote. <br /> Also,the safety data sheet(SDS)for this laundry detergent states that strong oxidizers are incompatible with the <br /> detergent, and the SDS shows the GHS05 hazardous pictogram for corrosive. The SDS was reviewed after the <br /> date of inspection. <br /> 4. The waste items listed below were found bagged and inside a tote labeled Flammable HW(IMG 7964-6): <br /> -A seemingly empty container of Pine Glo Ocean Antibacterial &Disinfectant Cleaner. There was no spill clean-up <br /> material found to account for the liquid missing from the container, and Jackie did not know what happened to the <br /> missing liquid. <br /> 5. The waste items listed below were found bagged and inside a tote labeled Toxic HW(IMG 7971-7991): <br /> -A seemingly empty 1 gallon container of Fabuloso Multi-Purpose Cleaner.There was no spill clean-up material <br /> found to account for the liquid missing from the container, and Jackie did not know what happened to the missing <br /> liquid. <br /> -A Revlon Color Silk permanent hair color kit,which contained a container of ammonia-free colorant, a container of <br /> cream color developer, and a package of after-color conditioner. Hydrogen peroxide is listed as the second <br /> ingredient in the developer.According to the PSC HW guidance poster and documents, items containing hydrogen <br /> peroxide are to be placed in the Oxidizer HW tote. The hazardous properties of the ammonia-free colorant and the <br /> after-color conditioner are unknown. <br /> -A Broadway Colors Quick Dry nail polish,which was labeled as flammable.According to the PSC HW guidance <br /> poster and documents,flammable items are to be placed in the Flammable HW tote. <br /> -A bag of Kingsford Original Charcoal Briquets. It could not be determined how the HW determination for the <br /> charcoal briquets was made, since the scanner did not indicate which HW tote the charcoal should be placed in, <br /> and the PSC HW poster and guidance documents did not list charcoal or a similar type of item. <br /> -A seemingly empty container of Purex Crystals In-Wash Fragrance Booster.There was no spill clean-up material <br /> found to account for the material missing from the container, and Jackie did not know what happened to the <br /> missing material. <br /> Page 3 of 4 <br />
