The following is an itemized list of hazardous waste violations that have not been <br /> addressed for Dollar General#13568 as of May 29, 2020. <br /> Open violations from July 31, 2019 inspection <br /> Violation#103-Failed to retain hazardous waste determination including waste analysis on site for 3 <br /> years. <br /> When asked how employees determine which of the differently categorized hazardous waste(HW)totes a waste <br /> item belongs in,Jackie Nash, Assistant Store Manager, said each item is scanned using a hand-held scanner, <br /> which will sometimes state which HW tote the item belongs in.Jackie indicated that when the scanner does not <br /> specify which HW tote an item belongs in, employees will read the item's label and ingredients, as well as the PSC <br /> HW guidance poster and documents,which provide product examples and key words for each of the HW <br /> categories,to determine which HW tote an item belongs in(see photos IMG 7952-3, 7988-9).When asked what <br /> employees do if they are still unsure about which HW tote a waste item belongs in, Jackie did not seem aware to <br /> call the store maintenance hotline phone number listed at the bottom of the PSC HW guidance documents. <br /> Additionally,Jackie and Steve Bohnak, District Manager,did not seem aware of Dollar General's(DG) Respond <br /> ticketing system to submit a notification to DG Corporate regarding the scanning system not providing a HW <br /> category for a particular waste item. <br /> The waste items listed below were observed in the HW totes, but at the time of inspection it could not be <br /> determined how the HW determinations for each of these items were made.The hand-held scanner did not specify <br /> which HW tote the items should be placed in, and the PSC guidance poster and documents did not list these <br /> types of items: <br /> 1.The waste item listed below was found bagged and inside a tote labeled Toxic HW(IMG 7937, 7940-1, 7988): <br /> -A tube of all purpose Krazy Glue. Steve Bohnak, District Manager, stated that he believed many adhesives or <br /> glues were flammable.A tube of The Original Super Glue was found in the HW tote for Flammable HW(IMG 7964, <br /> 7967-8).Also the PSC HW guidance document for Flammable HW pictured a tube of adhesive called Goop. <br /> 2.The waste items listed below were found bagged and inside a tote labeled Oxidizer HW(IMG 7959-61): <br /> -A leaking container of Arm&Hammer Laundry Detergent Plus Oxi Clean with Fade Defense Sparkling Waters. <br /> According to the PSC HW guidance poster and documents, laundry detergent is to be placed in a Toxic HW tote. <br /> Also,the safety data sheet(SDS)for this laundry detergent states that strong oxidizers are incompatible with the <br /> detergent, and the SDS shows the GHS05 hazardous pictogram for corrosive. The SDS was reviewed after the <br /> date of inspection. <br /> 3.The waste items listed below were found bagged and inside a tote labeled Toxic HW(IMG 7971, 7979-7982): <br /> -A Revlon Color Silk permanent hair color kit,which contained a container of ammonia-free colorant, a container of <br /> cream color developer, and a package of after-color conditioner. Hydrogen peroxide is listed as the second <br /> ingredient in the developer.According to the PSC HW guidance poster and documents, items containing hydrogen <br /> peroxide are to be placed in the Oxidizer HW tote.The hazardous properties of the ammonia-free colorant and the <br /> after-color conditioner are unknown. <br /> -A bag of Kingsford Original Charcoal Briquets. <br /> A generator shall make a hazardous waste determination and keep a record of any test results,waste analyses, or <br /> other determinations made in accordance with hazardous waste regulations for at least three years from the date <br /> that the waste was last disposed of. <br /> Immediately locate a copy of the hazardous waste determination made for the hazardous waste items listed above, <br /> ensure store employees are familiar with these waste determinations, and submit a copy to the EHD. <br /> If a copy is unavailable, immediately make a hazardous waste determination and begin managing the above listed <br /> waste under Title 22 hazardous waste regulations. Submit a statement to the EHD documenting the hazardous <br /> waste determination for each of the above listed waste items. <br /> Page 1 of 4 <br />
