f� CGRS <br /> Ji SOLUTIONS DELIVERED <br /> Police and Fire Department can be contacted bV using the numbers in Appendix A . <br /> Directions to the nearest Hospital can be found in Appendix B . <br /> SECTION 4. 0 <br /> 4A SITE CONTROL <br /> Site control is necessary to protect the personnel of CGRS our clients , subcontractors and the <br /> general public , and as a means of reducing exposure to contaminants and injuries . <br /> The risk of exposure to hazards and injuries will be reduced or eliminated by the use of the <br /> following methods where appropriate : <br /> • Setting up security of physical barriers at the work-site ; <br /> • Minimizing the number of personnel and equipment on -site consistent with effective <br /> operations ; <br /> • Establish work zones within the site ; <br /> • Establish control points to regulate access to work zone ; <br /> • Establish emergency evacuation routes and assembly areas . <br /> The health and safety plan will be discussed at the beginning of each day and the job foreman <br /> will review it each day . The Project Manager will review it each week . An emergency plan will <br /> be reviewed , along with routes to the nearest emergency facilities . Tailgate meetings will be <br /> conducted on site at the beginning of each workday and will include discussions of actions to be <br /> taken to minimize risks to on -site workers , visitors , and the public . <br /> 4 . 1 , 1 Site Hazard Evaluation = Personal Safety <br /> Personal Protection : Level of Protection A B C D <br /> Specific potential hazards may include , but are not limited to , heavy equipment operation , <br /> exposure to silica dust , hydrocarbon fumes , pinch points , overhead hazards , excessively loud <br /> equipment and pedestrian traffic . <br /> Modifications : <br /> 1 . All personnel must wear hard hat , safety shoes , bright colored vest or shirt and safety <br /> glasses . During disinfection events all personnel must wear hard hat , steel toed safety <br /> shoes , safety glasses , gloves , and hearing protection as required . <br /> 2 . Hearing protection must be worn if noise levels prevent normal conversation at a <br /> distance of three feet . <br /> 3 . No smoking , eating or drinking is allowed on site . <br /> 4 . No personnel are to enter or approach any excavation area where there is danger of wall <br /> collapse or confined space entry . Shoring , trench boxes and ladders shall be used in <br /> accordance with the CGRS Health and Safety Manual . <br /> 5 . Any personnel approaching machinery , heavy equipment , or excavation equipment shall <br /> do so only after establishing direct eye contact with the operator and acknowledgement <br /> to approach by the operator . <br /> 3 <br />
