OBSERVATION: The designated operator failed to check and note all required items on the visual inspection reports. The incorrect and missing information included the following from the inspection reports: -The 1/24/2020, 4/6/2020 and 12/24/2019, 9/23/2019, 8/27/2019, 7/29/2019 DO reports, The OWNER/ OPERATOR failed to provide a name, signature, and date within 48 hours of receiving the DO report under the ACKNOWLEDGMENT OF COMPLIANCE ISSUES section VI. -The 2/10/2020 and 3/9/2020 DO reports, did not include the owner/operator name in section VI. -The 10/21/2019, 9/23/2019, 8/27/2019, 7/29/2019, 6/3/2019 DO reports, The DO failed to add a description in section III to all components marked NA from section VII through XI. REGULATION GUIDANCE: During the visual inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. This is a minor violation. 215 CCR 2712(1) Failure to retain a copy of the permit to operate at the facility. OBSERVATION: A copy of the UST operating permit was not found on site. REGULATION GUIDANCE: A copy of the UST operating permit and all attachments and conditions shall be retained at the facility. CORRECTIVE ACTION: Immediately obtain a copy of the permit and retain it on site. This is a minor violation. 313 HSC 25290.1, 25290.2, 25291 Failure to construct, operate, and maintain primary containment as product-tight. OBSERVATION: Seepage was detected below the shear valve in under dispenser containment (UDC) 3/4 , indicating a leak in the primary containment. REGULATION GUIDANCE: All primary containment for the UST system must be product tight. CORRECTIVE ACTION: Immediately have a properly licensed, trained, and certified contractor repair or replace the failed component(s), under permit and inspection of the EHD if necessary.
