SITE CODE: 1270 <br /> 535 N UNION ST <br /> STOCKTON CA 95205 <br /> In a letter dated August 16, 1999 from San Joaquin County Public Health Services,"Environmental <br /> Health Division. (PHS/EHD),you were notified that the work to investigate the contamination at the <br /> above-referenced site proposed in the Site Assessment Work Plan dated May 12, 1998 prepared by <br /> Advanced GeoEnvironmental,Inc. (AGE), must commence by September 15, 1999. On August 20, <br /> 1999, in a telephone conversation with your tenant Mr. Randy Sutkin,he stated that the work cannot <br /> be performed because non-operating vehicles and other items are currently stored in that area of the site. <br /> He added that due to personal injuries, he is unable to move the items by September 15, 1999. <br /> PHS/EHD expressed that you are the responsible party and not Mr. Sutkin; it is your responsibility to <br /> comply with PHS/EHD directives. Mr. Sutkin explained that you have difficulties in mobility, and F <br /> must rely on him to handle the matter. Mr. Sutkin requested an extension_ of the date for <br /> implementation of the proposed works. <br /> The date for implementation of the proposed work has been extended to November 1, 1999. Submit a <br /> well permit application and$89 permit fee to PHS/EHD in advance of the work.
