It details how and why. <br /> 2-There's an actual "Well Permit Application"also. Use it as a master and copy it as you need it. Submit with a separate <br /> $89 check. <br /> 3-There's also a 'back-page'of the permit application, the"Licensed Contractor Declaration-LCD". This is for the driller <br /> only and only the driller, C-57, can sign it. I included it in case your driller doesn't have it/can't find it. OK to fax it to them. <br /> 4-The"Master File Record-MFR" identifies the land owner(on top), the facility(Geo-Pac) in the middle, and the bottom <br /> "3rd Party" is for a different entity that will receive our billing every 30 days. This is where you list AME and sign. This <br /> MFR is to be submitted with a 'pre-payment'of$267 (3 hours @ $89 per)for all 'new' billing parties. <br /> Once I get a copy of the CVRWQCB work plan approval letter and have ALL of the 4 items above, I can issue the permit. I <br /> recommend you 'fax'a copy of the permit application you plan to send to me to your driller at the time you send it to <br /> me..Gregg Drilling really likes to see it before they sign the LCD and send it to me. <br /> Typically I can issue the permit within 1-2 days after receipt at my desk..Allow 5-7 working days for it to get to my desk <br /> from the time you mail it. I will call you after I issue it so we may discuss drilling dates...I am very busy with other drillers <br /> now...nice weather, I guess. <br /> The permit and forms are 'ane-paged'docs. Margins on your printer may not be similiar to ours. The permit sometimes <br /> comes out on a page and a quarter. Just widen your right margin so it prints on a typical 8-1/2"X 11" piece of paper in <br /> case it gets it's bottom cut off. <br /> Good luck with the forms..Call if you have questions.
