The following is an itemized list of hazardous waste violations that have not been addressed for ALONSOS AUTO REPAIR as of March02, 2020. Open violations from January 13, 2020 inspection Violation#101 - Managed a hazardous waste without an identification number. OBSERVATION This business does not have an active hazardous waste ID number in the California Department of Toxic Substances Control(DTSC) database. EPA ID CAL000378844 is inactive in the DTSC database. In addition, the facility is using an incorrect EPA ID CAL000378843, which belongs to another business. REGULATION GUIDANCE: A hazardous waste generator shall not treat, store, dispose of, transport or offer for transportation, hazardous waste without having received an Environmental Protection Agency(EPA) Identification (ID) Number. A State EPA ID number is required for generators of less than 100 kg RCRA hazardous waste per month, less than 1 kg of acutely hazardous waste per month or any amount of non-RCRA hazardous waste. ID numbers are unique to a business and site location. CORRECTIVE ACTION: Go to the DTSC web site at https:Hdtsc.ca.gov/apply-for-hazardous-waste-epa-id-number/ to reactivate your State ID number. Immediately reactivate your ID number and submit evidence to the San Joaquin County Environmental Health Department within 30 days. ❑ This violation was corrected ❑ This violation will be corrected by (date): ❑ Supporting documents included Describe actions taken or will be taken to correct violation: Violation#114-Consolidated manifest receipts failed to contain complete information. OBSERVATION Consolidated manifest receipts found on-site for disposal of hazardous waste contained incomplete information. Two receipts were observed during the inspection dated 1-9-20 and 5-9-19 with an incorrect EPA ID number CAL000378843. The correct number is CAL000378844. REGULATION GUIDANCE: (4)All copies of each receipt shall contain all of the following information: (A) The name, address, identification number, contact person, and telephone number of the generator, and the signature of the generator or the generator's representative. (B) The date of the shipment. (C)The manifest number. (D)The volume or quantity of each waste stream received, its California and RCRA waste codes, the wastestream type listed in subdivision (c), and its proper shipping description, including the hazardous class and United Nations/North America(UN/NA) identification number, if applicable. (E)The name, address, and identification number of the authorized facility to which the hazardous waste will be transported. (F) The transporter's name, address, and identification number. (G) The driver's signature. (H)A statement, signed by the generator, certifying that the generator has established a program to reduce the volume or quantity and toxicity of the hazardous waste to the degree, as determined by the generator, to be economically practicable. CORRECTIVE ACTION: Immediately start using EPA ID CAL000378844 for all disposal of hazardous waste. Provide a corrective action statement and supporting documentation to the San Joaquin County Environmental Health Department within 30 days. Page 1 of 3
