Contact List and Information: <br /> All discharges should be reported to the Response Coordinator, Nick Chambers. Additional personnel <br /> should be contacted as described in Section 2.E of this Plan. <br /> In the event of a HAZARDOUS MATERIALS SPILL/RELEASE: <br /> 1. Evacuate immediate area, close doors upon exiting, warn others away, and initiate a CODE <br /> ORANGE by dialing: 1-1-1. <br /> *Do not attempt to respond beyond your level of training or certification. <br /> 2. Immediately contact one of the following individuals and provide the exact location and type <br /> of spill/release: <br /> Call "911" if emergency response is needed (fire, explosions, and other hazards) <br /> Personnel should have the following information ready—the Discharge Notification Form attached at <br /> the end of this Appendix should be filled out to ensure all information has been collected. <br /> a. Address of spill and phone number of facility <br /> b. Exact location of spill <br /> c. Name of person reporting release <br /> d. Source or cause of discharge <br /> e. Date and time of spill <br /> f. Hazardous materials/waste involved in release <br /> g. Quantity of material involved <br /> h. Damages or potential damages of spill <br /> i. Number of injuries if applicable <br /> j. Actions already taken to stop, or mitigate discharge <br /> k. Names of organizations and/or individuals already contacted <br />
