The applicant shall confirm with the Department of Health <br /> Services (DHS) , State of California, if a permit would be <br /> required for a hazardous waste facility or if a variance 0s,� <br /> from permit requirements would be allowed by DHS . b <br /> %W-p `V <br /> b. Contaminated soils subject to treatment shall betAanalyzedj <br /> reviewed, and approved by the San Joaquin Local Health <br /> District prior to and after treatment . Contaminated soils <br /> shall be restricted to soils impacted with unleaded gas, <br /> diesel, and unused motor oil . <br /> c . The above restriction shall be enforced until such time as <br /> the State Department of Health Services, Toxic Substances <br /> Control Division, issues a permit expanding the type of <br /> contaminated soil that can be treated at the facility (see <br /> attached State letter) . <br /> d. Submit a ground water monitoring program proposal to the <br /> San Joaquin Local Health District for review, permitting, <br /> and approval . ` <br /> e. Closure plans shall be ,-- *o the San Joaquin Local <br /> Health Districtval � ^� ^� 1 Rn - y-r �-roz <br /> -,f . No contaminated soils shall be received at the site until <br /> all required State and local permits are a <br /> obtained and the <br /> Use Permit conditions are met �0-� V Tol <br /> g. The operation shall not create a public nuisance. <br /> h. Application must be made to the San Joaquin County Air <br /> Pollution Control District for the necessary air pollution <br /> permits. <br /> i . The applicant shall file a "Report of Waste Discharge" <br /> with the California Regional Water Quality Control Board, <br /> Central Valley Region. <br /> j . A determination shall be made by the State Solid Waste <br /> Management Board as to whether or not the proposed project <br /> is(�a <br /> olidwastesite requiring a solid waste permit . <br /> 4 . Air Pollution Control District : <br /> a. An Authority to Construct permit is required. <br />
