4 <br />From: Robert Lopez [EHD] <rlopez@sjgov.org> <br />Sent: Tuesday, July 7, 2020 5:12 PM <br />To: Tim Beebe <TBeebe@WesternPeterbilt.com> <br />Cc: Ryan Severson <rseverson@WesternTruckCenter.com> <br />Subject: PR0521254; Western truck Center Hazardous Materials Program Compliance Review <br /> <br />Mr. Beebe, <br /> <br />I have reviewed the CERS submittal of June 29th and have found the following issues: <br /> <br /> Violation #5 – as discussed during the phone call, the new antifreeze was wrongly removed from the inventory. <br /> <br /> Violation #6, the site map does not show or address the following: <br /> <br />1. Access and exit points (from the street). <br />2. The storage/handling areas for new antifreeze and new diesel is not shown. <br />3. A legend to identify the yellow bar (probably a door) is used on the map and not included in the legend. <br />4. The topographical information is not accurate and does not show the entire facility and accurately <br />depict the area between the front of the structure and the street. <br /> <br />A site map shall contain a north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, <br />access and exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage areas, <br />and emergency response equipment. If a site map element is not applicable for your facility then list it on the map and <br />label as “NA". <br /> <br />Violations 5, 6, and 9 remain open.
