FOSTER <br /> FARMS . <br /> r It -` " ` ' Biosecurity Agreement <br /> Strict adherence to Foster Farms conceptual biosecurity policy of: Isolation , Traffic <br /> Control , and Sanitation , normally restricts visits from outside individuals to any <br /> Foster Farms poultry facility . In certain cases , the General Manager of the Division <br /> grants an exemption for essential visits after approval . <br /> In addition to requiring clean laundered garments at the hosting location ( clean boots , <br /> hairnets , and full coveralls) we are requiring all visitors to sign a biosecurity agreement <br /> and release form . <br /> If you feel that you are in violation or can 't comply with all of the conditions listed <br /> below we ask that you do not enter our facilities . If you meet the requirements please <br /> sign , date , and list the company you are representing below . <br /> Thank you for your cooperation . <br /> ■ I do not own any form of live poultry ( chickens , ducks , pheasants , quail , <br /> pigeons , guinea fowl , emu , ostrich , rhea, turkeys , etc) . <br /> ■ There is no live poultry on the premise or in direct contact with my primary <br /> residence . <br /> ■ I have not been physically in contact with any form of poultry (live) or poultry <br /> products (processing plant) within the previous 72 hours (* please see note) . <br /> ■ This visit to Foster Farms is my first commercial visit (i . e . have not been to <br /> other poultry customers) of the day . <br /> ■ I have not attended any game bird fights , exhibitions , or shows within the <br /> previous 72 hours ( * please see note) . <br /> ■ I have not attended any flea markets , swap meets , or auctions that had live <br /> poultry on the premise within the previous 72 hours (* please see note) . <br /> ■ I have not visited any family, friends , or associates property or residence that <br /> has live poultry on it within the previous 72 hours ( * please see note) . <br /> Note : The time restrictions for visitors wishing to visit a Foster Farms live bird facility <br /> and coming from a region or country that has been diagnosed with Exotic Newcastle <br /> Disease , Highly Pathogenic Avian Influenza , or Low Pathogenic Avian Influenza (H5 or <br /> 1-17 ) within the previous 12 months is increased 10 days . <br /> Note : Visitors may not enter the facility with any kind of camera or recording device , <br /> including mobile phones . <br /> Alzozv <br /> Signature Date <br /> pt c waft <br /> Company Representing <br />
