activities during the previous calendar year and shall <br /> include the following information: <br /> (1) The EPA identification number, name, and address <br /> of the facility; <br /> (2) The calendar year covered by the report; <br /> (3) The EPA identification number of each hazardous <br /> waste generator from which the facility received <br /> a hazardous waste during the year; for imported <br /> shipment, the report shall give the name and <br /> address of the foreign generator; <br /> (4) The description, quantity, and method of storage <br /> of each hazardous waste the facility received <br /> during the year, listed by EPA identification <br /> number of each generator; <br /> (5) Environmental monitoring data in accordance with <br /> Section 67195, Title 22, CAC; <br /> (6) The dates PCBs and PCB items are removed from <br /> service, and are placed into storage for <br /> disposal; <br /> (7) Total weight in kilogram of PCBs and PCB items in <br /> PCB containers with further breakdown of <br /> container contents such as liquids and <br /> capacitors; <br /> (8) Total number of PCB transformers and total weight <br /> in kilograms of any PCBs contained in the <br /> transformers; <br /> (9) Total number of PCB large high/low voltage <br /> capacitors; <br /> (10) Total number of PCB articles and equipment <br /> transferred not in PCB containers, and of PCB <br /> containers transferred to disposal facilities <br /> during the reporting cycle; and <br /> (11) A certification as required in item II.H of this <br /> permit. <br /> P.
