This version supersedes the previous <br />Checklist Only provided on 611812014. <br />This is a follow-up hazardous waste re -inspection. The following are Class II violations: 101, 108, 116, <br />122/125, 128, 301, 304, 404, 406, 407, 605/701, 703, 804/806/808/809, 812, and 9999. <br />101. This facility does not have a valid EPA ID number to manage hazardous waste at time of this <br />inspection. A hazardous waste generator shall not treat, store, dispose of, transport or offer for <br />transportation, hazardous waste without an EPA 1D number. Immediately obtain an EPA ID number to <br />manage hazardous waste and submit evidence to the EHD. Facility has since obtained an EPA ID on <br />7/3/2014; violation has been corrected. <br />108. The following wastes require hazardous waste determination: <br />Observed indoor (See legend on the attached map for building designation): <br />o Leaked oil in Building A (Photo Indoor #1, 2), and unknown liquid release in Building E (Photo <br />Indoor #7) <br />o Unknown substance released from bottle onto floor in Building A (Photo Indoor #3), <br />o Various bottles, buckets, and containers in multiple buildings which generator stated he does not <br />known, if they are still product or waste. (Photo Indoor#6,8,9,10,11,12,13,15,17,18,19) <br />o Corrosive solids on floor and On piping in Building D (Photo Indoor #4,5). <br />ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br />THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE <br />Inspector.
