Although you sent copies of manifest, I still need statement photos, or <br />paperwork addressing each of the violations listing at the bottom: <br />122 -failed to keep signed copy of manifests from the designated facility for 3 years <br />125 -failed to keep a copy of consolidated manifest for 3 years <br />128 -failed to retain copy of bill of lading for used oil filters for 3 years <br />407 -An empty HW or HM container (larger than 5 gallons) was not marked... <br />703 -Failed to comply with CFR 262.34 ... (this is the modified contingency plan) <br />804- Universal Waste in closed container <br />812 -failed to keep records of each shipment of universal waste <br />Attached is a Return to Compliance Form.
