Channel St.pdf <br />Hi Leon, <br />I reviewed your Return to Compliance Certification for the inspection conducted on 6/18/14 by Jeff Wong. Thank you for <br />addressing the majority of the violations. However, I still need a statement, photos or paperwork on what corrective <br />actions were taken or will be taken to address the following violations: <br />• 122 -failed to keep signed copy of manifests from the designated facility for 3 years <br />• 125 -failed to keep a copy of consolidated manifest for 3 years <br />• 128 -failed to retain copy of bill of lading for used oil filters for 3 years <br />• 407 -An empty HW or HM container (larger than 5 gallons) was not marked... <br />• 703 -Failed to comply with CFR 262.34 ... (this is the modified contingency plan) <br />• 804- Universal Waste in closed container <br />• 812 -failed to keep records of each shipment of universal waste <br />Refer to the attached inspection report for the more information on the each violation. Please send me the corrective <br />actions taken or will be taken for the above violations by 5/22/2015. Sign and date the return to compliance certification <br />form and attach it to your statement (see attachment). Any follow up of the return to compliance will be billed time <br />after 5/22/15.
