Identify your name, location and phone number. <br /> 2. Identify the address and phone number of the facility where the spill <br /> occurred. <br /> 3. Note the date and time of the spill. <br /> 4. Identify the location of the spill. <br /> 5. Identify what was spilled and, if known, the concentration, volume and <br /> chemical names of the material spilled. <br /> 6. Identify the source of the spill. <br /> 7. Describe all affected media, floor drains, soil, storm drains, pond (surface <br /> water) and the like). <br /> 8. Identify the cause of the spill, if known or obvious. <br /> 9. Identify any injuries or damage caused by the spill. <br /> 10. Identify if evacuation of personnel needs to take place. <br /> 11. Identify the immediate control and containment measures that have been <br /> or will be taken. <br /> Identify the name(s) of the personnel and/or organizations who have been contacted. <br /> -iii- <br />
