CONSENT <br />Consent to conduct inspection that involves: taking photographs, reviewing and copying records, <br />questioning personnel and inspecting hazardous waste handling areas. <br />Consent given by (name and title): Steve L. Manifests, Bills of Lading, LDR's and Exception Reports: <br />1 reviewed approximately ten manifests. Prior to conducting the inspection 1 reviewed the <br />Department's Manifest Database and observed two manifests (99631051 and 99552775) that the <br />Department did not have a record of receiving from the Treatment, Storage, and Disposal (TSD) facility. <br />Mr. Schmidt provided me with a copy of the manifests in question with the TSD facility's signature of <br />receipt. No SUSD manifest violations were observed. <br />b.
