A response was submitted by Pep Boys to the EHD on April 29, 2008. The following <br />information was not received or is not complete. <br />#46: Hazardous Waste Contingency Plan. The contingency plan submitted to the EHD does not include an <br />evacuation map, list of emergency equipment or list of local authority arrangements. A "Hazardous Waste <br />Contingency Plan" form is included with this letter. <br />#64: Hazardous Waste Tank Assessment. The EHD has not received the tank assessment document. The EHD is <br />not granting exemptions from this requirement. Guidance documents are included with this letter. <br />#69: Hazardous Waste Tank daily inspection records. The inspection records submitted to the EHD are not <br />complete. See Title 22 California Code of Regulations section 66265.195 for the inspection requirements. A copy <br />of this section is included with this letter. <br />Return to Compliance Certification: Send this document to the EHD when all violations are resolved. The form is <br />included with this letter. <br />Note: All EHD staff time associated with failing to comply, including the issuance of this letter, is billed at the <br />department hourly rate, currently $105.
